About the role:
Reporting to the Practice Manager, the successful candidate will provide individualised support to the client’s access to community, social and recreational activities ensuring delivery is of consistent and high quality. This support work can be provided at Bower Place or within the client’s home, school or community. You will provide varying levels of support coordination, taking into consideration an individual’s circumstance and level of complexity.
Duties:
Case management
Service delivery
Research a range of supports to suit individual needs across multiple providers
Work with funding levels associated with each client
Track and monitor billable hours
Coordinate meetings with clients
Maintain client records
Skills and experience:
Be flexible in nature and thinking
Have empathy and patience
Ability to multi-task
Sound problem solving skills
Exceptional organisational skills
Highly developed communication skills
Knowledge of NDIS (desirable)
Ability to liaise with a range of different stakeholders
A minimum level qualification Certificate IV in a Community or Human Services field
