Registered Nurse Home Care Coordinator - about the role:
The Registered Nurse Home Care Coordinator will report to and work closely with the Aged Care & Nursing Manager and be part of a dedicated team of health care professionals. You will be responsible for overseeing the care provided to Home Care Package (HCP) consumers and will lead the HCP fieldworker team to ensure quality services are delivered.
Duties:
- Conducting home care assessments and formulating care plans
- Developing, reviewing and monitoring care plans
- Assisting consumers to navigate the MyAgedCare portal
- Targeting and contacting prospective consumers via relevant channels
- Recommending new initiatives to ensure growth
- Liaising with other health professional to achieve the best outcomes for consumers
- Overseeing HCP fieldworkers in their delivery of services
Skills & experience:
The Registered Nurse Home Care Coordinator must be a Registered Nurse with current AHPRA registration and be experienced in aged care community services and the case management of consumers receiving home care services. Proficiency in Microsoft Office suite is required for this role and a Graduate Certificate in Aged Care Nursing would be an advantage.
Home Care Coordinator - about the role:
The Home Care Coordinator will report to the Registered Nurse Home Care Coordinator and be part of a dedicated team of health care professionals delivering Home Care Package services. You will be responsible for establishing and reviewing the care requirements of HCP consumers and ensuring quality care services are provided.
Duties:
- Formulating care plans in consultation with consumers and the Registered Nurse Home Care Coordinator
- Arranging the allocation of staff to provide services to each client
- Monitoring day to day service provision
- Assisting consumers to navigate MyAgedCare
- Liaising with other health professionals to achieve the best outcomes for clients
Skills & experience:
The Home Care Coordinator must have a Certificate III in Aged Care and proven experience in the administration of consumer directed care to the aged or disabled. Establishing, monitoring and reviewing services in a community setting is also highly desirable. You will also be proficient in Microsoft Office suite.
