This form should be completed by the employee along with the Host Employer Induction Checklist, to ensure that the employee is thoroughly inducted to the host employer’s workplace.

Please complete the WHS Self Assessment Checklist below and tick any boxes that are applicable:

The work areas free from clutter and obstruction?
The overhead lighting is adequate?
The lighting is able to be adjusted? (E.g.: with Venetian blinds) (Preferably the person (or the computer screen) should not be facing an uncovered window.)
The screen is free from any reflection and able to be seen easily i.e. no glare present (The screen should be perpendicular or vertical not tilted.)
The ventilation and thermal comfort are adequate?
The level of noise is conductive to concentration?
The stock material is stored safely without risk to persons using the area?
There is manual handling information available to workers and heavy items are not stored in high/inaccessible places?
The desk is suitable in size for the work being undertaken? (The desk should be 68cm – 72cm high. The dimensions should be a minimum of 90cm x 75cm for paperwork tasks or 115cm x 90cm for mixed clerical and computing tasks.) The chair has a lumbar support, adjustment of the height and the backrest and a 5 star castor base?

The depth of the seat is appropriate? (The thighs should be fully supported with 2-3-finger space behind the knees when seated and with the back fully supported by the chair.)
The chair be adjusted to the correct height needed for the correct keying position? (When seated the elbows should be at or slightly above the desk when arms are at the side of the body and shoulders relaxed.) (When keying, the forearms should be parallel with the floor and elbows bent at 90 degrees, with the wrist in the neutral position, not resting on the desk. The test position is with the fingers on the home keys (keys “asdf” left hand and “hjkl:” for the right hand.)
The chair be adjusted to the correct height so that the feet rest comfortably on the floor with knees bent at 90 degrees and thighs parallel with the floor? (If this cannot be achieved and the desk is not adjustable in height a footstool may be required.)
The top of the computer screen (toolbar) is at eye level when seated in the chair and looking straight ahead? (The spine should be neutral i.e. the natural “S” shape and symmetrical (i.e. directly in front of the keyboard and the screen equipment – not twisted.)
If glasses are worn which affect viewing of the screen, a neutral neck position can be maintained?
The keyboard is straight on the desk, approximately 3cm from the edge of the desk and in line with the body and the monitor?
The monitor is at least arms distance when seated in front?
The characters on the computer display are set at an appropriate size and colour for reading?
The image on the screen is steady?
The mouse has a scroll button feature and is used?
here a COVID-19 safe plan in place?
There are adequate washrooms and kitchens provided?
There are appropriate social distance spacing between desk chairs? (minimum 1.5 metres.)
There are face masks available if required?
There is access to appropriate hand washing facilities and/or hand sanitiser?
There are no observed broken plugs, sockets, switches in area to be used by on-hired worker and there are no observed cable trip hazards?
